jQuery / PHP form submit is not working

I am trying to create a simple form on my site where an email is sent to me, but I can not figure out why it is not working. When I press submit, I get the error message:

I am using localhost:8000 for my hosting site and am not sure if it has to do with that. I am using the WAMP server and have had no issues before, so I have no idea why it is not working now. Thank you for your help.
